Goku Grandpa Gohan

Goku's grandfather died after being crushed by Goku in his Great Ape form. The statement in Chapter 1 that he was already dead, the explanation of the Great Ape in Chapter 21, and Goku's realization in Chapter 233 that he was responsible are combined as the same past death. This is not a record that Goku intentionally killed his grandfather.

Goku Drum

The kick Goku delivers at the end of Chapter 154 and the destruction of Drum's head in Chapter 155 are treated as one continuous attack and one event. Tien is the one astonished that Goku defeated Drum in a single blow. Chapter 155, where the outcome can be confirmed, is used as the source.

Goku's temporary cardiac arrest and spontaneous return of his heartbeat

After Piccolo determines that Goku's heart has stopped and that he is dead, Goku groans and Yajirobe confirms that his heart has begun beating again. This site treats this brief cardiac arrest and spontaneous recovery separately from ordinary death and revival counts. It does not definitively classify the scene as a mere loss of consciousness or as one in which the original manga denies that he died. This note explains the counting standard.
